What are the names and locations in the universe of known black holes?

There are probably billions of black holes. Many astrophysicists think there's a black hole at the centre of most galaxies, and there are hundreds of billions of galaxies. There may be many other black holes besides these. The problem with finding out where a black hole is located is that, by definition, you cannot see black holes. People theorize that black holes might be located in specific places, but nobody knows for sure.
